COVER CARDIFF WEST - GP LINKED Integrated Multidisciplinary team providing care & treatment for adults of working age, whose daily functioning is being seriously impaired by mental health problems. Priority is given to people with serious mental health needs. Referrals mainly via the GP but accepted from other agencies - self referrals for those treated by secondary mental health care services during the 3 years previous. Building has limited disabled facilities - home visits can be arranged. Bilingual service can be arranged if requested in advance. Duty Worker available by telephone to give clinical advice.
