Multi-Disciplinary Team providing secondary mental health services for adults 18 to 65 yrs, living with a serious and enduring mental illness. Provides assessment and treatment service for adults in the Mental Health (Wales) Measure. Disciplines in the team include Social Work, Community Psychiatric Nurse, Occupational therapy, Clinical psychology, Psychiatry and Physiotherapy. Referrals - GP or self (if discharged within 3 years and have previously been allocated a care co-ordinator within last 3 years). We also accept referrals from other statutory and third section agencies and services.
