A/M out of hours. Emergency Social Services Team available. Crisis Assessment Service available 7 days a week at Prince Charles Hospital. Appointments normally necessary - A Duty Officer system operates 8.30am - 5pm. Multi Disciplinary Community Mental Health Team jointly managed by the Rhondda Cynon Taff County Borough Council and Cwm Taff Morgannwg Health Board. Assessment and Care Management Services for those clients who are referred to the Community Mental Health Team and meet the eligibility criteria. The Independent Living Service can be assessed by Assessment Staff who identify the need. There is a fairer charging assessment involved for those referred to the Independent Living Service who have a Community Support Worker and a possible charge for these services Referrals - We encourage referrals from GP`s and would advise that clients see their GP`s to request a referral to the CMHT or Primary Care Patients can contact CMHT directly. Self referrals if been discharged within 3 years We prioritise people with serious mental health problems and would refer those with less serious problems to other agencies such as counselling services.
