This is a multidisciplinary Community Mental Health Team. Services include: Social Workers, Community Psychiatric Nurses, and Occupational Therapy. Assessment and Care Management Services for those clients who are referred to the Community Mental Health Team and meet the eligibility criteria. Referrals via GP or self if discharged within 3 years of discharge Disabled Facilities - Ramp Access To Building, Adapted Toilets. Bi-Lingual Service Offered. Costs - Some Services are Charged for Eg: Domicillary Services.
