Cardiff and The Vale of Glamorgan Community Mental Health Team for Older People is a multi disciplinary team comprising of social workers, occupational and physio therapists, community psychiatric nurses, psychologists, social care officers, carer support officer and healthcare support workers. The team provide a range of assessments resulting in a wide variety of service provision including person centred mental health care and treatment, packages of care that includes at home, day care respite and respite or full time residential or nursing care. We also have links Mental Health Older Persons Services as well as other Cardiff and Vale University Health Board services such as The Memory Team, REACT, The Young-Onset Dementia Team and Care Home Liaison The Team provides a service to people over the age of sixty five years who have a late-onset functional mental illness resulting in complex mental health needs or to people of any age who have a progressive dementia with severe and unstable signs and symptoms associated with this. Referrals: UHB, Local Authorities, GP or self-referral if within 3 years of discharge.
