Covers North Powys Referrals via GP, other services; Patients self refer within 3 years of discharge date. Where severe/enduring mental illness or crisis is indicated, contact from 9am - 1pm Mon-fri. Range of therapeutic services such as one to one, cognitive therapy, occupational therapy, stress management of care. Assessment of social care needs. Out of hours service provision where indicated.
